Above 12 years old

• Original MyKad
• Original Birth Certificate / Citizenship Certificate (if applicable)
• Old passport (for renewal case)
• Police Report (for lost passport case)

• Original Mother / Father / Guardian MyKad
• Old passport (for renewal case)

Note: Applicants aged 13 years old and above are advised to renew their passport online. First time applications, children below 13-year-old and lost / damaged passport cases are NOT allowed to apply via this platform.
